Non-urgent advice: Social Prescribing Team
Social prescribing is an approach that connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.
Our Social Prescribers know everything that is going on locally to support patients. They can support with non-medical issues such as housing, benefits and debt advice, and can help patients to connect with local social and community support groups which all contribute to patients' health and wellbeing.

Non-urgent advice: Complex Needs Care Co-ordinator
Our Complex Needs CC supports patients with Learning Disabilities or Severe Mental Health (SMI) to attend their reviews to manage their health well. They can carry out Blood Tests, Blood Pressure checks, ECGs and COVID and Flu vaccines in a longer appointment for patients who need that support.

Non-urgent advice: Patient Experience Care Co-ordinator
Our Patient Experience Coordinator supports practices to acknowledge, investigate, and respond to patient complaints and concerns, and to report complaints in accordance with NHS guidance.
They manage the practices and PCN website and facebook pages, and is instrumental in establishing and organising the Patient Participation Groups for the PCN and individual groups across the practices. She also facilitates practice Digital Drop in mornings to support patients with online services.
Sasha also supports practices in contacting patients who do not attend (DNA) their appointments and need a reminder, those who attend the practices very reguarly and may benefit from additional support, and more elderly patients who have not attended for over 3 years to check they are looking after their health.
